Please include the following:

  • HCO Name: Organization Name
  • Subdomain Name: Subdomain name that was requested
  • New Authorized Representative's Name
  • Email address for new Authorized Representative
  • Phone number for new Authorized Representative

When this request is received, your DigiCert file will be updated by the next business day.
